Characteristics of HFNC Machine Therapy

Ⅰ. The HFNC machine can heat up, humidify and feed oxygen with a high total flow rate

 

The HFNC machine can administer CO2 with a higher concentration of heating and humidification through the nasal cannula. The total air flow can reach 60L/min, because the total air flow can be set to exceed the total respiratory peak flow level of most patients with inspiratory decline, thereby ensuring the stability of the oxygen concentration.

 

The warming and humidifying effect of HFNC machine can maintain the tracheal mucosa and improve the ability to remove microvilli from the mucosa. Effective airway humidification can dilute the secretions of the respiratory system, maintain the smoothness and dampness of the trachea, maintain all the normal functions of the respiratory system, and reasonably prevent complications such as lung infections.

 

Either way, inhaling a high total flow cyclone through a nasal catheter can increase the overall regional end-respiratory lung characteristic impedance by increasing the multi-functional residual capacity.

 

Ⅱ. Clinical medical application of HFNC machine

 

1. Used in patients with subacute hypoxic respiratory failure

 

Under the same PaCO2 level, the breathing rate of the patients treated with HFNC machine becomes slower, reminding that lung dead space can be reduced. The increase in lung dead space can lead to an increase in the mortality rate of patients with acute respiratory decline caused by hypoxemia and subacute respiratory distress syndrome.

 

2. Used in patients with respiratory failure who have not undergone tracheotomy

 

The included patients are mainly pulmonary interstitial fibrosis, pneumonia, chronic obstructive pulmonary disease, cancer, hematological malignant tumors and congestive heart failure patients. The median treatment time of HFNC machine is 30H. When patients with hypoxic respiratory distress refuse to undergo tracheotomy, the application of HFNC machine can show proper oxygenation and become another non-invasive ventilation method.
